Prevalence of cervical cancer by colposcopy & colposcopically directed biopsy among the VIA (+) women in a tertiary care hospital, Dhaka, Bangladesh

Abstract:

Background: Cervical cancer is the most common cancer among women & the most common cause of death in middle aged women. Invasive cervical cancers are preceeded by a long phase of precancerous lesion that can be detected early by screening & treated effectively by simple treatment. The major role of colposcopy is in guiding the diagnostic biopsy. Colposcopically directed biopsy is considered as histological diagnosis for the true underlying disease severity. This study was under taken to detect cervical cancer by colposcopy & guided biopsy among the VIA (+) women.
Objectives: Early detection of cervical cancer by colposcopy & directed biopsy among the VIA (+) cases & formulate the plan of management in order to reduce the morbidity & mortality.
Methodology: This cross sectional study was conducted in GOPD of Dhaka Medical College Hospital, Dhaka during the period of January 2013 to June 2013 among 50 VIA (+) patients. A copy of data collection sheet is attached.
Results: The data revealed that the peak age group of VIA (+) patients was within 36-45 years of age. Mean age was 37.12±9.93 SD, Range (18-60). Housewives were affected predominantly (88%), 50% were in lower income group and 54% were married between 16-25 years of age. About 80% respondants had 3 to 4 children, about 40% use hormonal contraceptives. Speculum examination revealed 64% of VIA positive women had healthy cervix. In Colposcopy, acetowhite lesion was found in upper lip 48% and in lower lip 52%, inflammatory changes found in 20% and preinvasive lesions in 72% cases. After punch biopsy followed by histopathology showed 43.48% inflammatory changes, 47.82% preinvasive lesions and 8.7% invasive cancer.
